About the Role:
This role requires an experienced Registered Nurse to join the Hospital Admission Risk Program (HARP) Care Coordination team at Northern Health, in a fixed term position.
This role is responsible for the delivery of safe, high quality care to clients with complex needs and/or chronic disease who are at risk of readmission to hospital.
To be successful in this role you will have: Minimum 5 years clinical experience Experience and excellent knowledge in care coordination or case management Clinical Leadership in the continuing care setting Demonstrated advanced clinical assessment, intervention planning and delivery skills. Demonstrated high level of knowledge of community, residential and acute/subacute services, that support clients with chronic disease and complex needs.
