About your role: Provide support services to determine the priority needs of senior Queenslanders, including the assessment of aged care requirements and a summary of assessment findings, goals and recommendations.Undertake Home Support assessments for Commonwealth Home Support Program services to meet the ongoing needs of the older person.Identify clients who would benefit from a Reablement Focused Program and action referrals to the appropriate services.Mandatory inclusion: Salary packagingAnnual incremental increasesCareer training and developmentWellness programsVaccine Preventable Diseases (VPD) evidence as required for your employment in accordance with legislation/government policy and Directives.
