Role details The Aged Care Assessment Team at the Mackay Base Hospital is seeking applications for a Home Support Assessor to join their team on a temporary basis.
Your opportunity To provide non-clinical home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT).
Under the Single Aged Care Assessment System, Home Support Assessors will work with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.
About the Team The Single Aged Care Assessment System is a multi-disciplinary team providing assessment services to older Queenslanders in the community.
The assessment system aims to simplify and improve access to aged care services.
The assessment framework is designed to streamline the assessment process and enhance the overall experience for our older Queenslanders.
