We need two Home and Community Care Coordinators, for our Macksville and Coffs Harbour offices. The Home & Community Care Coordinators are responsible for overseeing community programs including Home Care Packages (HCP), Commonwealth Home Support Services (CHSP), flexible care packages, private home support and brokerage services.
