The Shire of Manjimup is looking for an individual who has a commitment to enhancing the lives of older people and their family members/carers to join our friendly team at Manjimup Home and Community Care. This role is responsible for, but not limited to:
Assessing support needs, developing and implementing individual support plans for Home Care Package consumers;Consulting and liaising with the Clinical Care Coordinator in regard to all clinical matters;Undertaking regular home visits, and liaising with consumers and family members/carers regarding the implementation of support services provided;Providing regular reports to the multi-disciplinary team and maintaining active communications regarding consumers and service delivery;Identifying and managing risks to consumer safety;Reviewing support needs of the consumer twice yearly;Ensuring compliance with relevant program standards and reporting;Providing guidance and assistance to Support Workers (Aged and Disability);Sourcing equipment and materials for consumers, generating purchase orders and processing and reconciling invoices as required.The Person To be successful in this role you will need:
Certificate IV in Aged Care or higher, or a relevant qualification.Demonstrated experience in coordination and supervision of employees, as well as an ability to train and mentor employees and work in a multi-disciplinary team.Demonstrated commitment to the health and wellbeing of the lives of older people and people with disabilities and their family members/carers.Strong communication skills with an ability to work with people from a diverse range of cultures and backgrounds.Demonstrated literacy, numeracy, accuracy and attention to detail.A high level of initiative, coupled with excellent interpersonal skills allowing the maintenance and development of good relationships with local stakeholders.An excellent capacity to work within funding and budgetary confines.To Apply Head to www.manjimup.wa.gov.au for information about the position, including the position description, how to apply and information on the Shire of Manjimup and benefits of the local community.
