About the role As the Home Care Manager- Ipswich, you will be based at our SCCQ Raceview home and co-ordinate the implementation and delivery of the Commonwealth Home Support Programs (CHSP) for our clients in the Ipswich area.You'll ensure quality consumer care outcomes are met as well as providing support to both internal and external stakeholders associated with the Home Care programs for the area.What are the key responsibilities? Lead and manage a team of support workers and office support staff (Care Partners, Rostering / Scheduling Officers) Drive a culture of continuous improvement that has a strong customer service and solution focus, utilising customer insights and feedback Participate in the out-of-hours 'on-call' on a rostered basis Recruit staff as required Provide coaching, training, guidance and mentoring to staff to deliver quality services Ensure compliance against relevant Aged Care Standards Ensure all reporting and documentation requirements are met, including maintaining timely, accurate and efficient administrative and communication systems Ensure that complaints or incidents (SIRS) are reported, investigated, and responded to in accordance with SCC procedures, Aged Care Regulations and Quality Standards Meet Key Performance Indicators (KPI's) and budget as required Identify opportunities and implement strategies to increase Home Care Packages within the region Oversee rostering and scheduling of services to ensure good scheduling practices that maximise resources and drive efficiencies. Develop and maintain effective collaborative partnerships and strategic alliances with external stakeholders and the wider community to deliver best possible customer outcomes and leverage business opportunities

What you will bring to the role? Bachelor degree or similar relevant tertiary qualification in Community Services Experience in a similar role within Aged Care or the Health Services Sector Knowledge of Aged Care Standards and legislation Ability to lead and manage a team in a customer-focused service environment Capability to efficiently expand and develop services Relevant understanding and experience in the delivery of health services and/or community-based services as well as complex case management Demonstrated knowledge and experience in aged program management (assessment, coordination and budgets) and understanding the care needs of people within the aged sector Demonstrated ability to supervise and lead staff Demonstrated ability to plan, co-ordinate and organise own work and work of team members Strong knowledge of the relevant Acts and prescribed requirements
