We are a successful not for profit aged care provider, offering a comprehensive range of services from independent retirement living, home support, home care services, and residential care.
To meet our growing needs, we currently have an opportunity for a Full time Home Care Coordinator - Team Lead to join our Community Team.
The opportunity: Reporting to the Manager, Wellbeing and Home Care, this role is responsible for providing leadership and support, through collaborating and connecting across all home care functions.
Responsibilities: Pastoral responsibility to the Community Care Team, providing leadership, direction, and coaching.Provide consistent interpretation, common understanding and application of governing standards and guidelines across the Community Care function.Ensure a high and consistent level of customer service and support across our communities to enhance our understanding of needs and in turn their Aged Care service connections to afford 'More Good Days'.Undertake assessment and care planning activities within a philosophy of wellness / re-ablement, choice of service and in a manner that identifies the Client's needs, goals and expected outcomes.Respond to observations / reports of Client health changes and Client incidents, ensuring documentation is actioned in a timely manner and where appropriate.Coordination and reporting of Barossa Village Transitional Care Program.To be successful in this role, you will need: Alignment with Barossa Village Incorporated's Organisational Values.Excellent time management skills, with the ability to work effectively unsupervised.Ability to exercise a high level of initiative, leadership and problem-solving skills.Excellent customer service skills, with the ability to work with a broad range of Clients, staff and stakeholders.Prior experience in a Team Leader role.Police Check/Clearance not older than 12 months at appointment.Relevant experience working within a community care environment, preferably with a focus on the provision of aged care services.Comprehensive understanding of My Aged Care and the care system at large.Participate in the Annual Mandatory vaccination program.Demonstrated experience in Discovery Sessions, Care Planning and ongoing case management.Understanding of the Aged Care Quality Standards and other Government legislation that governs Community Aged Care Services.Understanding of the person-centred approach and developing person-centred care plans.If this role aligns with your skills, knowledge, experience, and passion, we would like to hear from you.
