Location: The Sutherland Shire, Miranda.

Key Responsibilities:
As a Care Coordinator, you will:

Coordinate and schedule operational activities in accordance with established care plans and organizational policies.
Liaise between clients, families, referral sources, and administrative staff to facilitate effective communication.
Build strong relationships with clients and their families, ensuring a positive and professional experience.
Educate clients and families on available services and benefits of care plans.
Monitor and assess the quality of care provided, identifying areas for improvement, and implementing changes as needed.
Assist with inside sales enquiries and contribute to business development opportunities.
Ensure compliance with regulatory agencies and safety standards.
Engage in general administrative tasks associated with the role

Qualifications and Skills:

Must have Case Management experience.
Relevant qualifications Cert IV, Diploma Nursing or higher highly regarded
Knowledge of the aged care industry and related issues.
Proven ability to assess, plan, implement, and evaluate care services.
Strong interpersonal and communication skills, both written and oral.
Computer literacy and analytical abilities.
Commitment to the Aged Care Sector and the well-being of seniors and vulnerable individuals.
Driver's License and comprehensively insured private vehicle.
