About the role: The Home Care Coordinator: Ensures that care needs are being met through high-quality service delivery Supports clients in developing or reviewing their Client Goal/Care Plan Coordinates and monitors the delivery of services, maintaining appropriate communication with carers and family members Mentors home care workers in the community for their benefit, and that of their clients.
This is a permanent, full-time position.
