About the role:  The Home Care Coordinator: Ensures that care needs are being met through high-quality service delivery Supports clients in developing or reviewing their Client Goal/Care Plan Coordinates and monitors the delivery of services, maintaining appropriate communication with carers and family members Mentors home care workers in the community for their benefit, and that of their clients.
This is a permanent, full-time position.
About you:   If you have a passion for elder care and a strong dedication to providing customer-focused care, this role may be for you.
If you: Can make sound judgements and develop high-level Client Goal plans based on your knowledge of Home and Community Care Enjoy working with a team to deliver services Have experience in mentoring others, then  APPLY NOW!
