About the role In this role, you will be responsible for coordinating our clients and their assessed needs and goal plans. You will ensure that care needs are being met through high-quality service delivery. Your tasks will include supporting clients in developing or reviewing their Client Goal/Care Plan, coordinating and monitoring the delivery of services, maintaining appropriate communication with carers and family members, and mentoring home care workers in the community.
This is a permanent, part time role – 2 days per fortnight. About you The ideal candidate for this position will have a passion for elder care and a strong dedication to providing customer-focused care. You will have the ability to make sound judgements and develop high-level Client Goal plans based on your knowledge of Home and Community Care. Additionally, you will enjoy working with a team to deliver services and have experience in mentoring others.
