Help at Home by Montefiore is a not-for-profit organisation which has been founded on Jewish values, specialising in the delivery of Home Care support across the Greater Sydney area. We provide the most committed team of carers to support our clients on a daily basis ensuring respect, choice and dignity is a priority for each individual client. Due to recent growth, our organisation is looking to expand our team of Care Coordinators and focus on clients based in the Greater Sydney region. Responsibilities include: Create and maintain the client's case management plan including conducting assessments, and determining the services needed in collaboration with the client. Proven experience working with complex clients, needs assessments and risk/safety assessments. Build and execute budgets which will support a client's home care package subsidy. Develop, implement, monitor and review case plans in collaboration with the client and families. Provide comprehensive assessments to identify client needs and requirements for clients using internal and external service networks. The role will offer: Security, growth and development Salary packaging 5 weeks leave per year Monthly ADO (accrued day off) A fantastic work culture and flexibility within the work environment To be successful you must: Be Tertiary qualified in Allied Health, Nursing, Disability, etc. Have a strong understanding of home care packages and My Aged Care portal. Have experience in setting up new clients including assessments, budget creation and general onboarding process. Have excellent attention to detail and ability to problem solve. Strong written and verbal communication skills. Have a customer-focused attitude and a passion for making a difference with the vulnerable and elderly.
