As a vital member of our team, you will oversee and facilitate the provision of high-quality care and services to our clients and participants.Hours: Monday to Friday 08:30am - 16.30pm Remuneration: Base + SuperannuationTravel: Covering Gold Coast including Tweed Shire TerritoryKey Responsibilities:Perform client care assessments and oversee ongoing care planningManage client compliance and maintain a case load of clientsCoordinate the delivery of personalised care tailored to clients' needs and care plans while adhering to allocated funding and budget requirementsEngage in general administrative tasks associated with the roleDesired Skills:Relevant qualifications Cert IV or Diploma Nursing highly regardedPrior experience in an aged care/NDIS/mental health care coordination role is advantageousFamiliarity with My Aged Care, Home Care Package, and NDIS systemsExperience in evaluating and planning care needs within the communityProficiency in using computer systems, including Microsoft Office 365, SharePoint, Outlook, and rostering softwareProven ability to manage incoming calls in a friendly and professional mannerDemonstrated capacity for planning, organisation, problem-solving, and solution-findingPositive attitude with a strong aptitude for teamworkOutstanding networking and consultation skills to establish relationships with external organisations for referralsExceptional customer service skills, both written and verbal, while maintaining confidentiality and privacy in accordance with company policiesCulture and Benefits:Rewarding work opportunities - making a difference in your local community;A very supportive, flexible, and positive team cultureEmployee rewards retail discount program;Competitive remuneration; andCareer progression and development opportunities will arise as the office expandsHow to apply:We are looking to fill this role promptly, seeking individuals eager for a position that offers significant job satisfaction and the opportunity to make a meaningful difference in our community.
