The Home Care Coordinator is responsible for supporting consumers of our expanding aged care operations.
Working closely with the project implementation lead and in collaboration with the Allied Health and Shared Services teams, the Home Care Coordinator is responsible to monitor, review and process incoming CHSP and Aged Care referrals in the My Aged Care portal, maintain close working relationships with external stakeholders, and assist with reporting obligations to our funders.
This involves liaising with consumers, families and carers to provide and monitor service coordination and scheduling for non-clinical components of support programs in accordance with care plans, maintaining a high quality of data and administration, and providing timely and accurate communication.
Your skills and knowledge will include: Working towards or completion of Certificate IV in Aged Care or related discipline, or equivalent experience in a similar role within aged care and/or allied health Demonstrated knowledge and understanding of the needs of consumers from various funding programs such as Home Care Packages, Commonwealth Home Support Program Excellent communication skills and demonstrated commitment to providing an exemplary level of customer service High level of experience and skill in computer applications such as MS Office, Cliniko, My Aged Care Ability to make sound judgements on competing priorities and communicate accordingly Demonstrated ability to provide input into developing processes, be flexible and adaptable in the early phases of a new project Benefits include: Hourly rate $39.56 per hour ($78,170 per annum) Salary packaging to increase your take home pay with $18,550 tax free each FBT year Birthday leave Journey Cover for your daily commute Rockingham office location with hybrid work options Flexible full time hours including a 9 day fortnight option Consideration given to part time (minimum 0.8 FTE) or part time job share Please contact Helen Griffiths, Team Leader Allied Health with any questions on 0434 612 397.
