The purpose of this position is to coordinate the daily operations of the Home Care Packages and/or Commonwealth Home Support programs.
This will be achieved by ensuring the provision of quality person centred care, promoting collaborative relationships amongst the health care team and external service providers and maintaining a culture of service delivery where the goals of the client inform our decisions across the continuum of care.
Duties: Coordinating all aspects of Home Care Packages and private clients within the regionOptimising care within clients identified care needs (liaising with Clinical and Allied Health and key stakeholders)Responsible for ongoing care planning, assessment and reviewsInvolvement in recruitment process for staffing requirementsManage a team of Home Care EmployeesAssist scheduling based on service needsAnswer new potential client enquiries regarding available service provision and pricingThings that work for us: Experience working within an aged care or community service essentialSupervisory/Management experience within an aged care/community setting (minimum 3 years desired)Cert III in Individual Support/Aged Care, Cert IV in Aged Care or higher; or Tertiary qualifications in the human services/ health care or management disciplines or a Diploma in Community Services Coordination desirableGood stakeholder management skills with ability to work collaboratively with the State Manager for the effective management of the programs;Experience and ability to maintain quality improvement activities, including the community service quality reporting;Experience and capacity to provide advanced level of case management and coordination of service provision to clients;An ability to support the well-being of the client through the professional, caring and efficient execution of required duties;An ability to observe and report changes in the client's status;An understanding of Aged Care Standards and consumer rights and responsibilities;Ability to work in a fast paced, time critical environment.Great time management, bedside and communication skills;Strong English written and verbal communication skillsIntermediate computer skills and previous experience with Client Management Systems (e.g.
