An exciting opportunity exists for a Home Care Assessor to join our Community Care Team, based in Dapto NSW.
About the roleThis position provides an important client-facing and referral role conducting assessments and completing support plans to register new HCP and CHSP clients in a timely manner and in consultation with the Scheduling Officer and Services Team.
The position also performs an important back-up and quality monitoring role by providing clients personal care, domestic assistance, respite, or social support as outlined within an agreed Support Plan which enables the clients to maintain their desired standards of personal care, hygiene and independence within their own home in accordance with the organisational protocols and procedures and Aged Care Standards.
This is a permanent position reporting to the Aged Care Senior Manager, working 9am to 5pm, Monday to Friday (37.5 hours per week).
The successful candidateTo be considered you must have:
Certificate III or IV in Home and Community Care and/or Disability or equivalent qualification.A minimum of 3 years work experience as a Care/Support worker.Experience in a supervisory or team leader position.Experience in completing CHSP & HCP assessments.Accurate and high level of administrative skills with intermediate level of computer proficiency, including use of Microsoft Office Suites and excellent data entry skills.High level of attention to detail and excellent written and verbal communication skills.COVID-19 Booster vaccinated, smart phone and reliable vehicle.RemunerationPosition is subject to award conditions under the Social, Community, Home Care and Disability Services Industry Award 2010 as Home Care Employee Grade 4.Uniform and laundering allowance provided as well as travel time between clients and kilometres.CareWays employees are also able to access salary packaging up to $15,899 tax free each year, meaning more money in your take home pay.How to ApplyIf you meet our criteria and you're looking for an employer who looks after their employees, CareWays is the place for you!
