About the Role As a Home Based Care Practitioner, you will work from a trauma-informed lens to provide direct case management to children and young people within the Foster Care and Kinship Care programs. You will also offer critical support and guidance to foster and kinship carers, ensuring that all involved feel empowered and supported.

This full-time, ongoing role involves applying a thorough understanding of relevant legislation and standards, conducting risk and needs assessments, and utilizing a range of service frameworks, including the Best Interest Case Practice Model, Looking After Children, TCI (Therapeutic Crisis Intervention), and Power to Kids frameworks.

Building trusting, supportive relationships with children, young people, families, and caregivers is central to this role. You will be expected to demonstrate a deep understanding of the challenges faced by these individuals, coupled with sound knowledge of the legislation affecting children, young people, and their families. To be successful in this role you must be able to work collaboratively with others, maintain professionalism and actively promote Brophy Family and Youth Services to the region.

Additionally, a solid grasp of trauma-informed care is essential, with opportunities for further training to enhance your expertise in this vital area of practice.

A full list of key duties is outlined in the Position Description, but includes responsibilities in: Engage with children, young people and their families using a variety of engagement strategies such as assertive engagement, outreach, and persistence and pacing. Undertake case management including intake, assessment, case/care planning, case coordination, supported referrals, case reviews and case closure. Facilitate, support and supervision access arrangement for children and young people. Support children, young people, their families and caregivers utilizing a variety of interventions and frameworks such as: the Best Interest and Looking After children frameworks, developmental theory, attachment and trauma theory, solution focused and task centered models and mediation. Do your skills include: Demonstrate a sound understanding of issues confronting disadvantaged children, young people and their families, particularly clients involved with the child protection system and out of home care program. An understanding of trauma informed practices. Demonstrated knowledge and understanding of the appropriate legislation, case management practice, and the ability to adhere to policy and procedures. An ability to maintain positive, effective working relationships with colleagues, peers, and relevant stakeholders in a wide range of government, business and community organisations. Are your credentials: A minimum qualification of a sector recognised and accredited Bachelor of Social Work or Diploma in Community Welfare Work or equivalent. (Qualifications must be approved by recognised professional bodies: ie. AASW, Community Services Council, APS etc.) A current Drivers Licence.
