About the team The Healthy Ageing team has been newly formed due to recent department realignment. The position will report to the Senior Coordinator Healthy Ageing. The team also includes Senior Community Partnerships Officer (who will oversee Moorleigh Community Village), Healthy Ageing Administration Officer (responsible for Council's 64 Independent Living Units and Community Bus hire program), and the Healthy Ageing and Active Officer (who will provide support and advice on a range of healthy and active ageing initiatives including support for senior citizens groups).
About the role As the Healthy Ageing Liaison Officer (HALO), you will play a vital role in fostering connections between Glen Eira City Council, community organizations, and health services to enhance the quality of life for older adults in our community. You will coordinate programs and services that promote healthy ageing, ensuring that older residents have access to the resources and support they need to lead active and fulfilling lives. In addition, the role will also be providing support to carers who play a critical role in our community for all age groups.
Other responsibilities include:
Actively engage with relevant internal and external stakeholders to build partnerships, promote the navigation service, and extend its reach to those who will benefit.
Support residents who require personalised information and practical assistance so they can navigate and make informed decisions about their care and support needs.
Assist with the development, promotion, and implementation of strategies to increase resident participation and reduce social isolation through community based and City of Glen Eira social connection options.
Demonstrate and drive a commitment to delivering a high-quality customer experience in all community interactions.
About you The ideal candidate will have a strong understanding of the challenges faced by older adults and a passion for fostering a healthy and supportive community, while also recognizing the role that caregivers play within it. With your strong interpersonal skills and effective listening skills, you will engage diverse stakeholders and drive positive change.
The following is also required:
Tertiary qualification in Allied Health or related discipline and/or relevant experience in Aged Care, Aged Care Assessments, or the Community Services sector.
Proven ability to communicate and work with a diverse range of people experiencing vulnerabilities, including but not limited to Elder abuse, mental health, social isolation, cognitive decline, and financial disadvantage.
Demonstrated understanding of Microsoft Office suite and literacy in utilising other digital channels and platforms for engagement and client management purposes.
An understanding of the Aged Care system, reforms and the opportunities and challenges faced by people as they age.
