About your role: Provide support services to determine the priority needs of senior Queenslanders, including the assessment of aged care requirements and a summary of assessment findings, goals and recommendations.
Undertake Home Support assessments for Commonwealth Home Support Program services to meet the ongoing needs of the older person.
Identify clients who would benefit from a Reablement Focused Program and action referrals to the appropriate services.
