The Diocesan Centre provides service support to our parishes and to other pastoral, welfare, community service and treasury functions of the Archdiocese of Hobart.Are you passionate about providing exceptional Healthcare? Are you committed to delivering high-quality care and fostering a supportive environment where you can make a positive impact on the lives of our Clergy community?As Clergy Healthcare Coordinator, you will be responsible for coordinating and providing support to working and retired Clergy of the Archdiocese of Hobart. This role coordinates access to health services and intervention as required by individual clergy.Clinical accountabilities include:Identify health issues by comprehensive health assessment;Implementation of individual care plans;Monitoring of health status with proactive interventions, education, and support;Maintain regular communication with clergy as negotiated with the individual;Management of health crisis where required;Coordination of appropriate services to deliver healthcare and support;Secure confidential and efficient management of medical records, individual health care summaries, emergency contact details, correspondence with primary healthcare providers, documentation of all contact with each clergy;Demonstrate professional practice via adherence to relevant professional competencies;Other duties as required within the scope of practice.Minimum Requirements:Registered Nurse with current AHPRA registration, with no conditions, undertakings, or reprimands, and compliance with nursing practice ethical and legal standards;Demonstrated experience within an acute or community care environment;Demonstrated advanced clinical knowledge and sound knowledge of the health care system and community health;Highly developed communication and interpersonal skills, and the ability to work collaboratively with other health professionals;Demonstrated critical thinking, problem solving, planning, and time management;Commitment to the identity and mission of the Archdiocese of Hobart, with an extensive understanding of the operation of the Catholic Church in Tasmania;Ability and willingness to undertake the relevant employee screening processes, including the provision of a National Police Criminal History Check satisfactory to the Archdiocese of Hobart and registration to Work with Vulnerable People (Tasmania).Inquiries:If you would like to know more about this exciting career opportunity, please contact Jo Rainbird, Senior People and Culture Business Partner on 0439 746 ***.Ready to apply?With guidance from the Position Description, please address each of the points in the selection criteria and let us know why you are the right person to fill this role.Closing date:Applications close Friday 1st November 2024.
