Our Practice Improvement Partners are key players in supporting change and implementing initiatives for improving patient care, increasing efficiency and promoting innovation in primary healthcare organisations. What you would be doing Every day you will have the opportunity to be the face of Primary Health Tasmania. With a strong emphasis on the primary healthcare sector, you will work collaboratively to strengthen integration between care teams and promote sustainable, high quality healthcare delivery. Here are some main areas where you will provide support: In-person visits, meetings, email and telephone support Develop and implement resources, and promote the use of digital health tools Support the development and implementation of quality improvement activities Coordinate networking opportunities and educational events Project delivery Support the use of MyMedicare and implementation of government incentives and systems such as Practice Incentive Program, Medicare Benefits Schedule and Chronic Disease Management (training will be provided for these programs) Here's what you'll look like You might have a background in health program planning/coordination - experience working with GPs or allied health professionals would be ideal You might have experience in practice management or senior administration Be a master relationship builder – able to engage and partner with stakeholders on our journey to achieve amazing results Ability to showcase and demonstrate health system tools, resources and initiatives Solid administration and report/brief writing skills You'll be a culture champion – understanding that every day, you play an important part of what makes Primary Health Tasmania a great place to work.
