About the Role:
Navigating the changes to our healthcare system can be intimidating and confusing for doctors and patients alike.
The Health Navigator's role is to make it easier.
You will start by developing trusted relationships with our patients and providers and will assist with various patient-facing, administrative, and logistical tasks so that doctors can focus on delivering high-quality medical care to their patients.
Navigators work for Main Street but go to work every day inside one of our partner clinics alongside clinic staff.
They meet with and call patients, enter data into our systems, contact the pharmacy to refill prescriptions, troubleshoot challenges with insurance coverage, and more.
Main Street's management and training teams will equip you with the training and tools you need to perform these duties.
You will:
Develop strong relationships with patients to assist them with their careMeet with patients during the check-in process, entering data into the Main Street software platform for reporting and tracking purposesCall patients to schedule an office visit or remind them to pick up their medicationOrder and schedule various procedures, tests, and screeningsIdentify and track progress on important gaps in patient careMaintain a record of patient interactions and communicate with providers using electronic health recordsEducate patients on their healthcare options, insurance benefits, and common medical conditionsHelp patients access various community resourcesHelp coordinate follow-up care after patients have been discharged from the hospitalDemonstrate compassion for patients, adaptability, attention to detail, patience, and an eagerness to collaborate with team membersRequirements for This Role: You are a self-starter who is comfortable working independentlyYou enjoy meeting new people and developing relationshipsYou bring a strong service mentality to your workYou love your community and want to see it thriveYou can skillfully explain the importance of key activities that make patients healthierYou are flexible and excited to tackle new challengesYou love solving problems and will take whatever initiative is required to solve themYou are comfortable using data to help inform decisions and activitiesYou are excited by the idea of working in a fast-paced organization where change is the normYou learn and apply new information quicklyYou are familiar with and comfortable using multiple software platformsWork a full-time 40-hour week; Monday-Friday 8 am to 5 pm (1 hour lunch)GED or High School DiplomaWe are an equal opportunity employer, indiscriminate of race, religion, ethnicity, national origin, citizenship, gender, gender identity, sexual orientation, age, veteran status, disability, genetic information, or any other protected characteristic.
