Latrobe Regional Health (LRH) currently has an exciting opportunity for an experienced and dedicated Health Independence Care Coordinator to join our growing team on an ongoing , full time or part time basis, dependant on successful candidate.
The primary purpose of the Health Independence Care Coordinator is to assess individual client needs and manage a case load of clients accepted and enrolled in one or more of the following Health Independence Programs:
Subacute Ambulatory Care Services (SACS), including Community Rehabilitation and Specialist Clinics. Transition Care Program (TCP). About you Ideal candidates for this role will have:
Relevant professional qualification as either a Registered Nurse (Division 1). Registration with the Australian Health Practitioner Regulation Authority (AHPRA). At least five (5) years postgraduate experience in the Acute Nursing field. Evidence of recent relevant professional development.
