About the Opportunity
This is an exciting opportunity to be part of a collaborative and multidisciplinary team that includes registered nurses, clinical nurse specialists, and clinical nurse consultants, all dedicated to addressing the unmet health and psychosocial needs of our community. You will have the opportunity to make a significant impact on the lives of patients with chronic diseases whose health and wellbeing are often compromised by complex psychosocial factors. In this role, you will combine your expertise with that of healthcare professionals from different disciplines, providing holistic, patient-centered care that not only addresses physical health but also supports patients in overcoming barriers to managing their conditions.
As a Social Worker/Occupational Therapist, you will provide specialized support to patients who are navigating chronic disease, often in the context of challenging psychosocial circumstances. You will assess patient needs, develop and implement care plans, and provide guidance to help them manage their conditions and access the resources necessary for better health outcomes. Your role will also involve advocating for patients, liaising with external stakeholders, and working closely with the team to ensure integrated and continuous care.
What You'll be Doing
The Keeping Well in Community Care Navigation and Coordination (KWIC CNCC) Program is an integrated care initiative within South Western Sydney Local Health District (SWSLHD) empowering clients with the tools, support and motivation needed to independently manage complex chronic and psychosocial conditions in the community. The primary objective of the program is to minimize the risk of avoidable hospital presentations and unplanned admissions, while expediting discharge from acute care settings. By facilitating and coordinating access to tailored care, non-clinical supports, and aiding informed/shared decision-making, we aim to comprehensively address a spectrum of unmet health and psychosocial needs to ensure clients can 'keep well in the community' for longer.
This position is responsible for providing specialist services to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. Expectations centre around maintaining manageable caseloads, while ensuring the provision of safe, quality care to clients with chronic diseases, and complex care needs. This requires close collaboration with fellow clinicians, and the multidisciplinary team to streamline service delivery. Using advanced skills, this position will exercise autonomy in decision making, and play a pivotal role in promoting health and wellbeing among clients and their families as part of the KWIC CNCC program.

Tertiary qualifications in social work with eligible registration through the AASW; or recognised Degree in Occupational Therapy or other qualification deemed appropriate by the employer, with registration with the Australian Health Practitioner Regulation Agency (AHPRA).Extensive post graduate experience working as a Social Worker or an Occupational Therapist in relevant therapeutic clinical areas, including a demonstrated understanding of chronic disease management, management of complex health conditions and the impact on wellbeing.Extensive demonstrated effective experience in intake, assessment, intervention, and evaluation of clinical outcomes with diverse communities and their families.Demonstrated experience in implementing home and community-based interventions to support health conditions and wellbeing.Proven commitment to a person-centred approach focusing on clients' strengths, preferences, and goals, with relevant skills associated with empowering clients and their families to actively engage in care planning and self-management.Demonstrated experience participating in the planning, implementation and evaluation of local service delivery incorporating the quality improvement process.Demonstrated high level organisational and time management skills.Excellent written and verbal communication skills and computer literacy in a range of programs.Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.To be eligible for permanent appointment to a position in NSW Health, you must have Australian citizenship or permanent Australian residency.
