This position will undertake the role of Assessor for the Commonwealth Aged Care Assessment Program, Regional Assessment program and to provide client-end assessment and support for the Geriatric Medicine Programs.The Aged Care Division delivers a range of clinical and non-clinical services across the entire Local Health District to support older people in their families to enable them to remain supported in their home. The aged care teams and programs have a strong commitment to advocacy, restorative care and reablement back to wellness and independence.
