Health Clinician – Aged Care Assessment Team Integrated Care Directorate
About the role This position will undertake the role of Assessor for the Commonwealth Aged Care Assessment Program, Regional Assessment program and to provide client-end assessment and support for the Geriatric Medicine Programs.
This role is responsible for undertaking holistic and comprehensive Aged Care Assessments in acute, community and residential aged care settings by developing person centred care plans to support clients until the point of effective referral to ongoing services.
You will be required to complete competency-based training relevant to the position and all mandatory training required by the Commonwealth, SNW health and the Local Health District (LHD).
