We're looking for a dedicated Health Care Navigator to join our team in Wagga Wagga, NSW (Wiradjuri Country) on a part-time basis up until June 2025!
In this role you will collaborate with our care coordinators in addressing the diverse social needs that influence health outcomes.
In this dynamic role, you will take a holistic approach, helping individuals overcome barriers related to finances, transportation, housing, legal issues, and social isolation.

Selection Criteria: The Essentials for Our Health Care Navigator role – Qualifications that Shine: You hold recognised credentials in Community Services, Human Services, Social Work, or a Health-related field.
You've got the knowledge to back up your passion.Advocacy Champion: You've got hands-on experience advocating for and connecting participants with local services (aged care, NDIS, community services) to help them hit their health and wellbeing goals.
You're their go-to resource!Person-Centred Pro: You know how to put the "person" in person-centred care!
You excel at working collaboratively with participants and their families to create tailored support plans.Health Detective: You have a keen understanding of the wider determinants of health—social, economic, and environmental factors—and how they impact communities.
