About the role
An exciting opportunity for an experienced Full-Time Home Care Packages - Case Manager to join our amazing team at Dungog and District Neighbourcare. The successful candidate will provide high-quality client services through client intake, assessment, care planning, referral, coordination, monitoring and case management.
Key responsibilities:

Visit clients in their homes, undertake client re-assessments, case management and coordination in consultation with the relevant stakeholders.
Develop support plans and implement the service delivery of these plans, with a strong emphasis on client choice.
Coordinating resources to provide the highest quality care and services to clients.
Liaise with brokerage services to allocate relevant/required services following client requirements and budget allocations.
Provide recommendations and referrals to Allied Health as required.
Ensure all client services adhere to the funding guidelines and are delivered within the appropriate timeframes.
Undertake regular reporting.

About you
In applying for this role, candidates must have a Diploma in Community Services or Case Management (or equivalent) and high-level experience in case management and care coordination for older people, people who live with disability, and people with dementia (and their carers).
Together with:

Previous experience working with older people and people who live with disability.
Knowledge of Consumer Directed Care, NDIS and Home Care Guidelines.
Demonstrated experience working with clients and carers to identify needs, risks and strengths to design a care plan, associated budget and service agreement.
The ability to work within a budget and meet agreed performance measures.
A commitment to enabling real client choice while managing client expectations.
Current NSW Driver Licence and a safe driving record.
A current NDIS Worker Check (or willingness to obtain).
A current Working with Children Check (or willingness to obtain).
