Our vision is for a Measurably Healthier Gippsland. We work closely with the primary health care system, identifying opportunities to improve health outcomes in Gippsland communities. We do this through health system improvement programs and by commissioning services to address local health needs via our regional commissioning role.
Ensure health issues are prioritised, understood, and invested in, using local strengths;Build and support local capability in primary health care professionals;Enable transparent communication and information exchange across the primary health care sector;Improve access in primary health services particularly for those at risk of poor health outcomes, and where gaps exist in hard-to-reach locations.WHO WE ARE LOOKING FOR:Do you have experience in managing and delivering complex projects, and development, implementation and evaluation of health programs, with a high level of stakeholder engagement skills?
We currently have a full-time (1.0FTE) career opportunity available as the Project Coordinator. This position oversees the implementation, tracking, and monitoring of funded targeted projects and key initiatives for Gippsland PHN within a dedicated project management team consisting of the Project Coordinator and Project Officers, utilising a standardised project management approach.
Reporting to the Manager Primary Care Response, the Project Coordinator role actively contributes to operational planning, culture development, and continuous improvement in policies, systems, programs, and processes.
It is expected that as a Project Coordinator you will engage and nurture partnerships with a range of primary health professionals and sectors, including general practice, allied health, nursing and midwifery, and other primary health providers to provide quality access to primary care services; and provide leadership for health services to adopt evidence-based best practices and emerging rural sustainable models of care, whilst embedding efficient systems and processes.
We are looking for an astute, agile, dynamic, and outcomes-focused team member. Use your skills to complete work packages including but not limited to:
Supporting Design projects in line with incoming schedules/grant guidelines;Implement activities and maintain project plans;Monitor project progress, including deliverables of the project, budget and scope, and maintain project governance;Create project status reports for the leadership team and Executive;Ensure compliance with the funding guidelines and ensure alignment with Gippsland PHN guidelines;Develop communication/engagement plans to support and enhance the delivery of projects in collaboration with other Gippsland PHN teams;Engage with and build relationships with project stakeholders;Collate data and write professional reports relating to the projects;Work with the Digital, Data & Insights team to monitor and evaluate programs achievements, challenges, impacts, and outcomes;Report to funders and work with external evaluators;Coordinate and lead Project Officer/s to ensure deliverables are on track and risks are raised;Manage contracts of commissioned services or consultants who are delivering services for the project;Respond to population health planning outcomes to prioritise locally and regionally relevant activities;Support other core activities; andProvide considered advice to the Manager Primary Care Response ensuring strategic and operational risks, challenges, opportunities, and key issues are identified.WHAT WE OFFER:Attractive remuneration;Salary packaging;Flexible working environments with a choice of being based at one of our three office locations (Traralgon, Wonthaggi, or Bairnsdale), with a mix of working from home environments by agreement;A great organisational culture; andThe chance for you to make a real difference to the health outcomes in our community.HOW TO APPLY:To view the Key Selection Criteria, please visit our Careers Page at: https://gphn.org.au/career/ and click on the position description hyperlink within the role advertisement and navigate to page 4.
