Our five-year Strategic Plan is focused on growing as an innovative, responsive and better-connected health service provider for all Australians.Watch stories about our people, patients and the communities we serve at finestcare.com.auWe believe our people – all of our people – keep the Royal Flying Doctor Service flying.About this OpportunityWorking within our dynamic multidisciplinary Primary Health Care Team as our next General Practice Nurse, you will be inspired to provide comprehensive complex patient care for people living in remote and isolated areas of the far North and West of South Australia.  With a focus on health promotion, illness prevention and community development, you will coordinate a holistic care approach to achieve better health outcomes for our diverse range of patients.  Based in Port Augusta, this role will also provide you with the opportunity to cover Primary Health Team FIFO rosters, enabling you to build on your scope of practice and experience all that the RFDS has to offer!In this role you will:- Competently perform clinical nursing duties identified as being required in the preparation and execution and monitoring of care plans according to best available evidence e.g. Immunisations and other injections; wound management, ECG's and collection of pathology samples. Assist doctors with minor medical procedures.- Use clinical software and evidence to identify and target the people within our patient cohort with Chronic Disease and Complex Care needs who are most likely to benefit standardised, coordinated approach (care plan).- In partnership with the RFDS SA/NT doctors and remote area nurses assess patients' health-related risks and needs and develop care plans centred on patients' needs and preferences and support patients in learning more about their conditions and self- management.- Connect patients to appropriate specialists, allied health and support services.- Manage the Immunisation portfolio, Rheumatic Heart Disease portfolio and Preventing and Controlling Infections portfolio.About YouYou are:- An AHPRA registered nurse with extensive community nursing experience- A passionate advocate for Primary Health Care, including patient education and health promotion in regional areas- Computer savvy (experience across Best Practice and PenCS CAT4 would be ideal) and in possession of high level clinical documentation skills- In possession of ALS accreditation coupled with knowledge of emergency resuscitation techniques. This will enable you to assist with caring for critically ill or injured patients should the need arise- A confident, independent practitioner, you also enjoy working as part of a multi-disciplinary team- A strong communicator who can adapt easily to the needs of a variety of stakeholders- Seeking to work in a dynamic role with an organisation that makes a difference to the lives of rural and remote Australians!Our BenefitsAs part of our team, you can expect to be provided with challenging, interesting work and be supported to achieve your best.
