About The Role Full-time position based in Geelong Join our team as a Home Based Care (HBC) Case Manager! In this vital role, you'll receive referrals, assess needs, and coordinate placements with the support of your Coordinator. You'll develop and implement personalised plans for each child, ensuring their needs are met while supervising and supporting foster families and, when appropriate, their birth families. You'll meet all statutory requirements, write reports, and provide regular supervision to caregivers. We believe in a collaborative approach, so you'll participate in team discussions, fostering open communication.
