You will also need to work in collaboration with families and community stakeholders to assess and work towards permanency for each young person. This will include helping them to build connections in the community as well as assisting them to independent living. This role will involve; Comprehensive case management for Aboriginal young people, families, and foster/kinship carers. Collaborate with families and community stakeholders to assess and strive towards permanency for each young person. Assist in building community connections and facilitating the transition to independent living. They are looking for candidates with case management experience with vulnerable children and families. The requirements for this role include; Minimum of a Diploma in Community Welfare and/or demonstrated relevant experience. Theoretical and practical skills in the areas of risk assessment and case management of the target group. Possession of a current Driver's License valid in NSW is required.
