Position Title - Support Worker About the Role - The purpose of this role is to provide appropriate leadership to Support workers and participants, ensuring a safe, participant-centered plan by empowering the clients and team to achieve their NDIS goal and improve their quality of life at home and in the community.
Job Responsibilities - Assisting people in daily tasks such as dressing, bathing, grooming, tooth brushing, medicine administration, and restroom use.
Supporting the client's physical and mental health with routine checks, administering prescriptions and measuring blood pressure.
Providing a safe environment for the client by monitoring their surroundings for potential dangers such as fire and tripping hazards.
Collaborating with other healthcare providers to coordinate clients' care plans.
Providing diverse activities such as arts & crafts or games for clients who can't go outside.
Supporting clients socially and arranging visits or outings in the community.
Identifying needs and developing plans for improving the quality of life for those suffering from disabilities or mental health issues.

Act with respect for individual rights to freedom of expression, self-determination, and decision-making in accordance with relevant laws and conventions Monitor staff daily performance to ensure quality care for the participants and support is delivered in a safe environment.
Ensure new staff is trained well with participant care needs, daily routine, and risk assessment. Completing a shadow shift checklist Ensuring Staff completed shift duties and reported any concerns around the house to TL.
Act with integrity, honesty, and transparency respect the privacy of people with disability Completing a monthly checklist Environment Cleaning Schedule.
Report any hazards at the house to the admin Coordinator and complete the hazard report form.
Develop a fortnightly menu plan according to the participant's health and diet requirements.
Skills and Qualifications - Certificate III or IV in Disability is desirable Committed to providing exceptional support to the participants and staff across all aspects – written, phone, and face-toface.
The ability to communicate clearly and concisely, varying communication styles for participants, staff, and external stakeholder's network Excellent attention to detail and written skills when communicating with others, both internally and externally Willingness to assist and support others as required and get on with team members Time management/ Empathy towards the role.
