TLC provides supported accommodation, day programs including community participation and active ageing, daily activities and in-home care.

About the role The focus and purpose of this role is to provide person-centred support to assist Tender Loving Care clients reach their full potential, enhancing their quality of life, through whole of life programs focused on the individual.
House Leads work to empower clients and assist them to achieve independence in their daily lives.
 

The House Lead needs to work with each client and CSW in ways that reflect the individuals communication style, health needs and physical supports.  House Leads will support clients and teams to meaningfully contribute to the community through a range of activities.
 
In addition, the House Lead is responsible for identifying community engagement opportunities, providing person centred coaching and mentoring and program development and redesign.


 
About you: Relevant experience supporting people with an disability and/ or children and young people in a related area/ field Awareness of the rights of people with disabilities and/or multiple disabilities; Strong commitment to inclusion of people with disabilities within the community; Ability to support and motivate Tender Loving Care clients to ensure that they can attempt to reach their full potential at all times; Possess good knowledge of the disability service standards and ability to "live" them; Demonstrated interpersonal skills with the ability to communicate and relate to people we support and their families and friends. Demonstrated ability to work as part of a team and contribute to team goals Good level of general fitness and ability to complete manual handling tasks including some lifting; Computer literacy skills, including being a competent user of MS Applications including Word, Outlook, Excel and experience using databases; Competent administrative and documentation skills Be flexible, highly motivated, possess emotional intelligence, maturity, a sense of humour and willingness to subscribe to the Tender Loving Care mission and values; Maintain current state driver's licence; Hold a current First Aid certificate Key responsibilities:  Understand the needs and experiences of the people we support and uphold their rights Empower individuals and teams to provide clients with the opportunity to make informed decisions and choices. Work effectively with our clients and teams strengths, abilities and needs, in consultation with the individual and relevant stakeholders. Actively seek to understand the people we support and engage with community to create meaningful relationships Develop and maintain safe and professional relationships with clients and supported staff Assist, support and develop independence in activities of daily living, not limited to personal care, hygiene and health care i.e. eating, drinking, toileting and dressing. Ensure medication is administered to clients according to the client and organisational guidelines. Maintaining the communication book and checking Tender Loving Care site and Tender Loving Care email on a daily basis; Ensuring safe, lawful and responsible operation of any Tender Loving Care motor vehicles or other equipment.
