About this role
The Case Coordinator will support the team in achieving set targets and meeting organisational and department goals. The Case Coordinator will focus on supporting the Team Leaders and maintaining strong relationships with participants, their families, and key stakeholders to ensure best practice services align with individual goals and plans. This role also includes management and development of staff, administrative duties including service agreements, medication system oversight, critical incident reporting, and formal supervision.
Salary: SCHADS Award Level 4 - Pay Point 1
Main Duties & Performance Goals
Oversee and implement Individual plans with a person-centred approach.
Assist in the intake and induction of new participants.
Manage and monitor staff, including behaviour plans and incident reporting.
Build relationships with stakeholders for positive participant outcomes.
Attend relevant NDIS and specialist appointments.
Review participant profiles and introduce new activities.
Facilitate communication and advocate for participants' needs.
Supervise and manage Team Leaders.
Provide training and mentoring to Team Leaders.
Monitor and document the success of IPP goal implementation.
Report issues and risks to the General Manager.
Review medication administration and ensure accurate recording.
Oversee medication charts and manage changes in CTARS.
Ensure workplace health and safety, report incidents, and adhere to safe work practices.
Maintain documentation, reporting, and safeguarding requirements.
Qualifications & Experience
At least Certificate IV in Individual Support or equivalent required.
NDIS Worker Screening and NDIS Worker Orientation Module.
Current First Aid & CPR.
Current NDIS Worker Screening Check.
Current national police check (or willing to obtain).
Open Driver licence.
Evidence of right to work in Australia.
Experience with any of the following will be highly regarded - case coordination and the ability to develop, implement, monitor, and evaluate programs for people with disabilities.
Experience in working with Indigenous Australians and people from culturally and linguistically diverse backgrounds.
Well-developed written and oral communication skills, particularly an ability to communicate effectively with a wide range of individuals and organizations.
