About The Role The Salvation Army Family Violence Services NT provides a range of programs supporting adults and children experiencing domestic and family violence, including crisis accommodation, outreach, and counselling.
We are seeking a Permanent Full-time Family Violence Case Manager/ Practitioner to join us in the case management team to support families staying at a newly built refuge for families escaping family violence.
The FV refuge provides up to 3 months of secure accommodation and intensive case management support for victim survivors of family violence.
Referrals can be made directly from the Police, THFC, other FDV and community services, and individuals can self-refer.
The Family Violence Case Manager/Practitioner is responsible for providing case management support to adult and child victim survivors of family violence, involving ongoing risk assessment, safety planning, case planning, providing psycho-education, information and advice, and completing referrals to other services to provide holistic wrap-around supports and continuation of support for victim-survivors.
Salary and conditions are in accordance with the SCHADS Level 5 Award.
Key Responsibilities Effective relationships are established with clients that encourage self-determination and participation and uphold the principles of the National Family Violence Model of Care.
Specialist FV Case management and crisis support is provided to clients in accordance with TSA case management models and standards of practice.
Ongoing assessment is made of the victim survivor's issues, needs, and circumstances from initial intake to exit.
Conduct ongoing family and domestic violence risk assessment and safety planning with clients using agreed tools.
Individual case plans are developed in collaboration with victim survivors and include a range of interventions that address the needs or goals of clients.
Case plans are implemented and actively led by the victim survivor.
Victim survivors are assisted to access holistic support services.
The ideal candidate will bring A tertiary qualification in Social Work is essential.
Experience working in the family violence sector is desirable.
Experience in specialist family violence and/or child wellbeing work is highly desirable.
Demonstrated case management experience and skills.
Highly developed time management and work prioritisation skills with an ability to work autonomously with minimal supervision required.
Advanced computer skills including experience in data and record management.
Ability and willingness to be on call.
Relevant child protection requirements are obtained, for example, Ochre Card.
