KAMS has successfully delivered high-quality, accessible comprehensive primary health care services over its 30 years of operation and has provided innovation and national leadership in areas such as health information management and evidence-based best practice in primary health care.
About the OpportunityKAMS now has an opportunity for a Family Partnership Worker to join their friendly, multidisciplinary team in Halls Creek WA, on a part-time basis. The Family Partnership Worker (FPW) is integral to the successful implementation of the Australian Nurse Family Partnership Program (ANFPP). The Family Partnership Worker is responsible for maintaining high level standards of community practice, fostering acceptance of the ANFPP model in the community and observing the Primary Health Care Services' policies and guidelines.
Some of your key responsibilities will include (but will not be limited to):
Awareness and understanding of KAMS strategic planContribute in developing and implementing strategies to meet goals set in the provision of comprehensive primary health care to the Indigenous people of the Kimberley Region.Participate in Reflective Practice to align practice to ANFPP model.Maintain alignment with client-centered principles including modeling therapeutic relationships, change theory, self-efficacy, client-centered, strength-based, and solution-focused approaches.Contribute to team knowledge about referral resources that are appropriate to the goals of women participating in the program.Please note: Aboriginal and/or Torres Strait Islander descent is a genuine requirement of this role (50D)
