We are currently looking for a Fulltime Home Care Package Advisor to join our QLD team.
Job Purpose:
The Home Care Package Advisor performs an integral role in assisting in individualised services being delivered to customers.
Duties and Responsibilities: Assist in customer service schedules including service planning, confirmation and short notice placements or cancellations.Liaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal and other regulatory bodies.Ensure care plans are monitored and reviewed on a regular and ongoing basis and in response to changes in customer needs.Ensure services are delivered in line with the person-centred and consumer-directed principles.Assist customers to access other alternative services where the services are no longer meeting their needs.Liaise with relevant community-based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives.Monitor customers' budgets in accordance with program guidelines.Act on care need recommendations by Health Care Practitioners.Identify and participate in personal professional development.Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.Communication with key stakeholders, internally and externally.Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.Any other duties as required to meet the ongoing needs of the organization.Selection Criteria: Ability to work flexibly to respond to changing customers' needs.Ability to coordinate services according to relevant program guidelines.Experience of working in a multi-disciplinary team.Ability to develop and maintain links and supportive networks with specialist services.Experience maintaining computerised customer information systems.High level administrative and organisational skills.High level of written and verbal communication skills.Demonstrated ability to contribute positively to the team.Current and clear Criminal Record Check.Current Working with Children Check.Current NDIS Worker Screening Check.Current Drivers Licence.Qualifications in Community Services, Human Services, Nursing or equivalent.Previous experience in Aged Care or Community Services.Previous experience with Home Care Packages.Previous experience as an NDIS Coordinator.Bachelor of Nursing.Next steps:
