HammondCare's residential services are provided across two homes, including Caulfield Village, a unique dementia specific environment.

Whilst having the autonomy to work with residents to achieve the best care outcomes, you will also be part of a broader team of in-house Allied Health professionals working alongside and supporting frontline teams.Together you will deliver resident-centred, evidence-based care in an environment that reduces excess disability, promotes meaningful engagement in life and strives to implement continuous quality improvement consistent with the values and ethics of HammondCare and international best practice.Essence of the roleProvision of effective and timely physiotherapy or exercise physiology to residents and the appropriate support and advice to their representatives and the HammondCare team, to meet individual needs and preferences.Collaborate closely with residents, their representatives, staff and the broader Allied Health Team to assess complex care needs, set care goals, and provide treatment and interventions to deliver the best possible outcomes for residents.Participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment.Participate in quality improvement projects and continuing professional development.About youCommitment to engage and align with HammondCare's Mission, Motivation and Mission in Action.A tertiary qualification in Physiotherapy and registration as a Physiotherapist with AHPRA or a tertiary qualification in Exercise Physiology and accreditation as an Exercise Physiologist with ESSA.Practical experience in supporting people with varied and changing needs.A current driver's license, safe driving record, with a willingness to drive the organisation's minivan (C class license).Effective communication and interpersonal skills (written and verbal).Demonstrated effective time management skills and self-initiation.Demonstrated ability to work independently and as part of a multidisciplinary team.About usHammondCare is a not-for-profit, values-based organisation that is passionate about improving the quality of care for people in need.
