This position will be combined with another Physiotherapist position in the Chronic Pain Clinic working 2 days per week.
Both of these roles combine to create a full time position 80 hours per fortnight with an ADO.
HARP-DMT is an interdisciplinary team that provide home based services to clients with chronic cardiac and respiratory disease.
This is a great opportunity for a physiotherapist who enjoys providing home and community based care to maximise a patient's independence.
The HARP-DMT is a friendly, supportive team that has a well-developed interdisciplinary approach.
The chronic Pain Clinic is a continuously developing service for the region with a focus on the assessment, care planning and management of people experiencing chronic pain.
Team members include a medical specialist, clinical psychologist, occupational therapist, physiotherapist, exercise physiologist, dietitian, allied health assistant, enrolled nurse and Clinical Nurse Coordinator.
You will have: Broad based clinical experience in the area of assessment and management of people experiencing complex health issues, chronic pain/chronic cardiac and respiratory disease Demonstrated knowledge and understanding of service delivery across the health sector.
High level of independent practice as well as the ability to work collaboratively as a team member.
Excellent communication and interpersonal skills as well as an understanding of an evidence based approach to person centred care are critical to the role.
An ability to work collaboratively within an interdisciplinary team.
