About this role The successful applicant will work under our tested multidisciplinary model of care, that has significant, proven benefits for getting our clients onto their best health pathway.
Our model uses a comprehensive assessment battery and case conferencing process to inform targeted prescriptions, with the client journey culminating in ongoing exercise and service participation.
Key Responsibilities include but are not limited to: applying quality diagnostics and treatment skills, to the needs and wants of our clients contributing positively to multi-disciplinary care planning for our clients demonstrated experience in person-centred practice, service continuous improvement, evidence based treatment and service delivery engaging in case conferencing and problem-solving of complex care needs proactively delivering evidence-based reablement pathways that ensure client physical, social and mental health benefits demonstrated willingness to mentor students and to increase your personal skill set For further details regarding the scope of the role, please refer to the website or the attached position description.
About you We are looking for people who will be the difference in the lives of our residents and clients.
The successful candidate will have the following skills and experience: Must be an Accredited Exercise Physiologist with an appropriate tertiary level qualification and current registration with Exercise and Sports Science Australia Have experience delivering health literacy to clients Posses well-developed technical skills to design, implement and evaluate safe and effective exercise interventions for older people Extensive knowledge and understanding of the effects of exercise interventions in managing chronic and complex clinical presentations Ability to teach, coach and motivate clients, using models of behaviour change, scientific evidence and clinical reasoning Have strong organisational and time management skills as well as being able to demonstrate initiative to work autonomously with minimal supervision Exceptional interpersonal skills including the ability to build and maintain strong internal and external stakeholder relationships Ability to maintain concise documentation Proficient computer skills including the ability to use the Google Suite and various in-house databases Commitment to professional self-development in the field of clinical exercise.
