About us Anyinginyi Health Aboriginal Corporation (AHAC) is a multi-disciplinary organisation which provides primary health care services to the Aboriginal people of Tennant Creek and the surrounding Barkly region, dedicated to 'Closing the Gap' in Aboriginal health within remote Australia.
Anyinginyi consists of five different sections (Health Services, Business Services, Public Health Unit, Sports and Recreation, and Piliyintinji-Ki Stronger Families) which allow them to have a holistic approach to health ensuring that clients' physical and emotional health and wellbeing is given the utmost priority.
Anyinginyi's mission is to be a provider of high quality holistic primary health care services featuring prevention and treatment in the Barkly region in a culturally responsive way.
About the Role This role will primarily work to address health care needs for Aboriginal peoples and their families residing in Anyinginyi Health's four remote Barkly communities/outstation service delivery areas (Wogyala, Corella Creek, Mungkarta and Nuramin including; other smaller Barkly outstations that may have Anyinginyi Health clients and families residing from time to time).
The Rural Remote AHP OR RN will work within a small team providing community-based approach to the Barkly remote communities and outstations in accordance with the Anyinginyi Health, Primary Health Care Delivery Model.
Based in Tennant Creek.
You will be responsible for the day-to-day operations, plan and provide regular remote outreach community screening, health assessments and individual or family's needs per CARPA standard treatment manual.
You will be required to participate in the continuity of patient care and ensure better community outcomes within the role.
The occupant of this position will be expected to comply with and demonstrate a positive commitment to the highest level of achievement in Equal Employment Opportunity, Work Health & Safety, Code of Conduct, Quality Improvement, Performance Management, Client and to ensure Focus and Confidentiality throughout the course of their duties.
Some of your responsibilities may include, but are not limited to : Plan and provide regular remote outreach community screening, health assessments, and treatment for Individual clients and their families as per the Northern Territory CARPA standard treatment manual.
Support and promote teamwork through open communication, collaboration and contribute to a positive workplace culture.
Receive and follow up on 81300/10950 EPC referrals to provide specified follow up services for clients and families who have completed preventative health assessments and/or GPMP/TCA.
Advocate for rights and needs of individuals, families, and communities on health-related issues.
Participate in the implementation of Anyinginyi's health programs.
Provide referrals and bookings for both primary health care health assessments, completion of health checks, CDMP's and referrals of them including allied health specialist appointments.
Facilitate telehealth where required to promote continuity of patient care.
Participate and comply with all Work Health & Safety responsibilities.
Actively maintain knowledge of the application of the MBS; ensure all EPC follow up visits.
Collaborate with AHAC clinics and CDC to provide follow up treatment and management of preventable diseases.
Lead and provide health education awareness for population health and notifiable disease outbreaks.
About you To be considered for this position, you must Possess current registration as an Aboriginal Health Practitioner and or Registered Nurse and evidenced understanding of the Primary Health Care system and key issues relevant to closing the gap in health outcomes for Aboriginal and Torres Strait Islander people.
Experience working with an Aboriginal organisation or communities will be highly regarded but is not essential, provided you are able to demonstrate a clear understanding of the issues pertaining to Aboriginal Primary Health Care and the social determinants of health in our surrounding communities.
An exceptional communicator with outstanding interpersonal skills is needed to tailor your approach to the needs of each family and possess the ability to identify their strengths and build effective relationships with our clients.
