Role The Inclusion Support Coordinator is responsible for supporting the Inclusive Education Leader in the implementation and delivery of Disability Inclusion program.
Responsibilities Support the Inclusion Education Leader in the implementation of the Disability Inclusion program in accordance with DET requirements and the 3-tiered funding model. Liaise with key personnel, including teaching and education support staff, responsible for the delivery of educational adjustments and targeted support for Tiers 2 and 3 students. Support Disability Inclusion Profile processes, including identification of eligible students and preparation of applications to secure optimal levels of funding and support. Help manage the transition of current PSD funded students to a Disability Inclusion Profile. Provide support and guidance in the development of individual education plans (IEPs) for students with functional needs. Attend SSGs for students. Build staff capacity to understand, develop and implement adjustments to support students. Support the Leading Teacher - Inclusive Education and the Inclusion Team to prepare and gather supporting information for Disability Inclusion profile meetings. Maintain a close working relationship with Sub School, Inclusion and Wellbeing Teams. Participate as a member of the Student Wellbeing Committee.
