Provide a safe space to improve client outcomes with a trusted mental health service provider. Enjoy a diverse role in crisis management, outreach psychosocial support and collaboration with clinical teams and external services. Part-time (.7) contract position until 30/6/2025, with possibility for extension.
About You
Are you passionate about helping individuals living with mental illness and motivated to improve the lives of those experiencing crisis? Are you empathetic and gentle in your communication? Are you reliable and consistent in your practice, as well as being able to demonstrate the following attributes?
Patience, emotional resilience, effective listening skills & an adaptable communication style. Capable to advocate for others and build sustainable support networks. Effective time management that is flexible and adaptable to prioritise and action critical incidents. Accurate in your reporting with fastidious attention to detail. Prior experience in mental health, case management (community or clinical setting), or qualification/s in mental health, social science or other relevant field.
About The Role
Hospital Outreach Post-suicidal Engagement (HOPE) delivers psychosocial support to individuals who are at-risk of suicide, or have recently made an attempt to take their own life.
HOPE is facilitated across a series of domains including community and social services, housing, finance, family support, cultural and spiritual support, education and employment as well as advocacy for all of the above.
The provision of psychosocial support to eligible participants of HOPE is delivered as assertive outreach. Assertive outreach is a proactive approach to delivering support to non-engaged or at-risk clients where support is offered in different settings at times that are mutually suitable. Effective assertive outreach is characterised by sustained and persistent efforts to reach clients of HOPE at any suitable location and to work with them in partnership to improve their safety and quality of life.
This position operates on a rotating fortnightly roster, including morning shifts and evening shifts. The program hours are between 8:30am-9:00pm Monday-Friday, and 8:30am-5:00pm Saturday, Sunday and public holidays.
The role entails:
Build professional relationships with at-risk individuals, their families, and other stakeholders, including collaborating with allied health services and clinical staff. Support clients with psychosocial tasks, assist them in setting goals and creating safety plans, while identifying risks and implementing crisis management strategies. Refer clients to external services for ongoing support. Focus on producing high-quality and precise reports on client wellbeing, participation, and plans for clinical reviews. Operate in a multidisciplinary and risk-averse environment, emphasizing the importance of process adherence, clear communication, and accuracy in a challenging setting.
This is a part-time role. The successful applicant must be available to work the following roster:
Week 1: Tuesday 12:30pm - 9pm, Thursday 12:30pm - 9pm, Friday 12:30pm - 9pm.
Week 2: Tuesday 8:30am - 5pm, Thursday 8:30am - 5pm, Friday 8:30am - 5pm, Saturday 8:30am - 5pm.
It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children Check, work rights and possess a current & valid driver's licence.
We take appropriate actions to ensure that our staff and the people we support are as safe as possible. You will be expected to adhere to all OHS standards, requirements and processes, including (but not limited to) wearing appropriate PPE and regular hygiene and cleanliness practices.
