Job Description Qualified Social Worker, Knowsley Council, Adults Hospital Discharge Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.
The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.
The responsibilities will include:
Complete continuing health care assessment alongside health colleagues. Based within the hospital discharge team which will include hospital assessment/discharge. Review of 24-hour care placements. Report writing. Liaise with other professionals. 36 hours per week (flexible by negotiation). 2 Month Temporary Contract. Start date subject to DBS and reference clearance.
