This short-term program, lasting up to 12 weeks, is designed for individuals who are typically eligible for ongoing care in a residential aged care facility but have the potential to regain their independence. In this rewarding role, you will provide personalised care and support to help our clients enhance their physical, cognitive, and emotional well-being, empowering them to thrive in the comfort of their own homes. You'll play a crucial part in fostering recovery and promoting autonomy, making a meaningful difference in the lives of older Australians during a critical time in their journey. Where you'll be working: This casual position will see you working as part of the Transitional Aged Care team, alongside Enrolled Nurses, Registered Nurses, Physiotherapists, Occupational Therapists and Domestic Assistants. You will be responsible for the assessment and care of clients to assist them to achieve their goals. This program runs as a 7 day/week service so will include some weekend work. Face-to-face training will be provided to transition into the role and support will be available from RN's, EN's, NUM and Service Manager in the position. About you: You are a motivated Enrolled Nurse who can take initiative in community settings. You are skilled in interpersonal and communication, allowing for effective consultation with a multi-disciplinary team. You are capable of collaborating effectively as part of a dynamic team.
