Current Position: Enrolled Nurse We are seeking an Enrolled Nurse who is motivated, reliable, and passionate about aged care to work in Northam and surrounding towns. Delivering services under a range of funding sources including My Aged Care and NDIS, and Private Pay Clients, the primary responsibility is to provide quality in-home care and assistance to our large client base.
The successful applicant will be working with our care team, liaising with Clients, Family Members, Staff, and a wide range of Stakeholders to ensure services are provided in accordance with client's needs, goals, and preferences. Assessment, planning, and reviewing care plans as required, services are Client focused and Consumer directed at all times with quality outcomes achieved using a flexible, responsive, creative and collaborative approach.
In addition to case management, this role will manage inside and outside sales enquiries, develop and maintain new and existing referral sources.
Qualifications Demonstrated knowledge and experience in a similar role for coordinating consumer directed care services to Govt funded HCP clients. Demonstrated knowledge of the Aged Care Act. Have advanced communication skills and the ability to model a positive culture and strong values through a professional service. Demonstrated understanding of the changing and diverse needs of clients. Reliable fully insured vehicle. Drivers License. Australian National Police check. First Aid and CPR. Previous experience with rostering is highly desirable. Experience with managing staff. Excellent networking and consultation skills to develop new referral sources and relationships with appropriate external organisations.
