About the role The Home Care Coordinator (EN) is responsible for supporting person centered services for clients which are responsive to the clients assessed needs, within the Home Care portfolio, as well as the provision of EN nursing services within the Scope of Practice.
The Home Care Coordinator (EN) is the central point of call for a client to problem solve and escalate issues (as required), ensuring the smooth delivery of services according to the clients assessed needs, service plan and budget.
Key Responsibilities of this role also include: Conduct onboardings, reviews, assessments and develop budgets for clients in the region, resulting in clients having a current and holistic assessment and a service plan which manages all risks associated with their assessed needs, within their service budget and with a health promoting focus Ensure client services are set up in a timely manner Provide nursing services in line with the Scope of Practice and Competency Standards for Enrolled Nurses Coordinate, and when required, lead, daily check in meetings Completion of administrative tasks associated with the region For further details regarding the scope of the role, please refer to our website or the attached job description.

The successful candidate will have the following skills and experience: Diploma of Nursing with current and unrestricted AHPRA registration as an Enrolled Nurse Current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009) Current and unrestricted driver's (C class) licence Demonstrated experience working as an Enrolled Nurse in a Community / Home Care or Residential Aged Care setting, as well as experience in client support roles Sound interpersonal and communication skills, including a high level of proficiency in the English language (both written and verbal), with demonstrated ability to communicate effectively with older people with complex needs Time management skills to autonomously manage own workload Effective decision making ability, including the ability to identify potential risks and escalate as appropriate within the organisation Proficient computer and keyboard skills, including the use of community aged care systems/processes (eg, MyAged Care, ACAT, RAS etc) About us Southern Cross Care is one of Australia's leading charitable aged care, health and retirement living service providers.
