Your responsibilities include liaising with clients, family members, staff, and stakeholders to ensure services align with clients' needs, goals, and preferences.
This role involves assessment, planning, and review of care plans, with a focus on client-centric and consumer-directed services.
In addition to case management, you will manage inside and outside sales inquiries, as well as develop and maintain new and existing referral sources.
QualificationsDemonstrated knowledge and experience in a similar role for coordinating consumer directed care services to Govt funded HCP clients.Demonstrated knowledge of the Aged Care Act.Current registration with AHPRADiploma in Enrolled NursingAdvanced communication skills and the ability to model a positive culture and strong values through a professional service.Demonstrated understanding of the changing and diverse needs of clients.Reliable fully insured vehicleDrivers LicenseAustralian National Police checkFirst Aid and CPRPrevious experience with rostering is highly desirableExcellent networking and consultation skills to develop new referral sources and relationships with appropriate external organisations.Preferred SkillsClinical Skills: Proficiency in patient assessment, wound care, administering medications, and using medical equipment.Patient Care: Ability to provide compassionate, patient-centered care, including monitoring and managing patient health, and responding to their needs effectively.Communication: Strong verbal and written communication skills for interacting with patients, families, and healthcare team members.Critical Thinking: Capability to make sound clinical judgments and decisions in emergency situations or when dealing with complex patient conditions.Attention to Detail: Precision in documenting patient information, following protocols, and ensuring accurate administration of treatments.Problem-Solving: Ability to identify issues and implement appropriate solutions in a fast-paced healthcare environment.Adaptability: Flexibility to adjust to changing conditions, patient needs, and work environments.Knowledge of Healthcare Regulations: Familiarity with healthcare laws, ethical practices, and infection control procedures.Education and Certification: Diploma or degree in Enrolled Nursing and current registration with AHPRA.Cultural Competence: Ability to provide care that respects the diverse backgrounds and needs of patients from various cultural groups.
