Chesterfield Royal Hospital NHS Foundation TrustWe have an exciting opportunity to recruit to the post of 'Enhanced Clinical Practitioner' to work in the community looking after our frail and elderly patients.
The role is a full-time permanent position working 37.5 hours per week.Must have completed both level 7 modules of APACS/CDM and Non-Medical Prescribing qualification and completion of E-portfolio to be uplifted from a trainee band 6 to a qualified band 7.
Or - able to evidence all aspects of person specification if already in the same role.
Main Duties of the JobTo provide expert enhanced clinical management for patients who are frail and/or with multiple long-term conditions who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission, or unnecessary length of hospital stay.
To provide enhanced level clinical assessment, make a diagnosis, initiate appropriate clinical treatment, refer appropriately and lead to the coordination of care, working with partner agencies and key professionals.
Investigate and diagnose an unwell individual with frailty and or long-term conditions in their own homes and in care homes.
Reduction of unplanned admissions when appropriate.
Management of those with long-term conditions including frailty.
High-quality care to patients and families.
Managing complex medical conditions including long-term conditions and frailty.
Carry out acute one-off visits when required (within own skill set).
Dealing with challenging behaviour.
Working with existing resources.
Dealing with vulnerable clients and families.
Developing own clinical skills.

Person SpecificationEducation and QualificationsRGN or HCPC accredited degree that can complete the additional post-registration training to V300 Independent non-medical prescribers.Registered with NMC (Nursing and Midwifery Council).Evidence of post-registration clinical professional development.Enhanced clinical decision-making and patient examination skills.Personal Attributes and QualitiesAble to recognise own limitations, identify learning needs and act on them appropriately.Ability to work autonomously and use own initiative.Ability to assess and manage risk when working with individuals.Experience of working with patients in their own homes or a care setting.Flexible approach to the evolving nature of the role.Ability to work collaboratively in a team.Drive to provide excellent integrated patient care.Ability to sometimes work under pressure to meet deadlines.Adaptable approach to meet service needs.ExperienceHigh level of understanding regarding long-term conditions agenda and wider NHS agenda.Working autonomously in an advanced nursing role.Community experience.Previous experience of leadership/management.Experience in a primary care setting.Experience with SystmOne.Experience or training in personalised care and support planning.Skills and KnowledgeExcellent communication skills both written and verbal.Management of risk and health and safety and safeguarding adults and children procedures and ability to implement.High level clinical skills relevant to long-term conditions and the management of chronic disease management and frailty.Advanced clinical practice skills.Management of patients with complex needs.Accountability of own role and other roles in a primary care setting.Knowledge of practical application of clinical governance.Willingness to work flexible hours when required to meet work demands.Access to own transport.Ability to travel across the locality on a regular basis.Diagnostic and clinical assessment skills.Teaching and mentorship in a clinical setting.Knowledge of population health issues.Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary: £46,148 to £52,809 a year per annum pro rata dependent upon experience.
