You will be providing short to medium term case management for consumers presenting with complex mental health needs. Under the guidance of the Team Manager and Team Consultant Psychiatrist, and in conjunction with team members, the Enhanced Care Clinician will provide and coordinate short to medium term additional specialist, targeted therapeutic interventions to consumers that are receiving case management services.
Interventions provided will be aimed at supporting psychosocial recovery, fostering individualised recovery, independence, and improved health outcomes for consumers that have been identified as having severe and complex needs. Interventions provided could include, but not be limited to, cognitive behavioural therapy, family therapy, peer support, interventions focused on activity/vocation, or additional treatment interventions to meet physical health or drug and/or alcohol needs.
Classifications for this role will range from:

The Continuing Care Team provides ongoing specialist treatment and support to meet the identified needs of people with a serious mental illness. The multidisciplinary team provides a range of clinical and therapeutic interventions using the recovery-focused Strengths Model of Recovery-Oriented Practice.
